Well, yes, they do. The needle has to penetrate the skin and inject/place the ink below the epidermis of the skin. This hurts and causes bleeding plus if the area tattooed is not kept clean and disinfected you can get an infection or rash which will really hurt as well as damage the tattoo.

and that he didn't have a licence or a professional
but she got one anyway
she got a star on each foot
but one got infected because he didn't clean the tattoo thing right
and she nearly died because of the infection
Tattoos hurt, but it also depends on your tolerance level for pain and the area of the body you'll will have your tattoo.
