Well, yes, they do. The needle has to penetrate the skin and inject/place the ink below the epidermis of the skin. This hurts and causes bleeding plus if the area tattooed is not kept clean and disinfected you can get an infection or rash which will really hurt as well as damage the tattoo.

but she got one anyway

Tattoos hurt, but it also depends on your tolerance level for pain and the area of the body you'll will have your tattoo.
